Field | Description |
340B ID | A unique identifier assigned by HRSA to each covered entity participating in the 340B Drug Pricing Program. |
Manufacturer Name | All manufacturers with an existing designation will be present. |
Kind | Retail or Limited Distribution Networks (LDNs), which are networks utilized by pharmaceutical manufacturers to control the distribution of specialty medications. |
Next Designation Time | “Next Designation Time” states the date your entity may make a new designation. If it has been over 12 months since the designation was made, this will state today's date – meaning eligible for a updated designation. However, if your covered entity wishes to maintain their same designation, no action is required. |
Pharmacy ID | Pharmacy ID is a unique, national identification number assigned by the National Council for Prescription Drug Programs (NCPDP) to every licensed pharmacy in the United States and its territories. |
Pharmacy Name | Retail contract pharmacy name as reflected by HRSA. |
Pharmacy Address | Retail contract pharmacy address as reflected by HRSA. |
Note: Contract pharmacy designations can generally be updated once every 12 months from the date the designation was created. If a contract pharmacy relationship is terminated in HRSA OPAIS or the pharmacy closes before the 12-month period ends, the covered entity may be eligible to submit a new designation.
