How to Find a HRSA 340B ID

Help finding a HRSA 340B ID utilizing the HRSA database

This article provides details on how to find a HRSA (Health Resources and Services Administration) 340B ID.

A 340B ID is a unique identifier provided by HRSA assigned to each covered entity parent or child site (e.g., child site, subdivisions, or sub-grantees).

To find a covered entity's 340B ID:

There are two ways to search across Covered Entities:

  1. The Search dropdown from the top panel of the page allows searches for active covered entities, contract pharmacies, and manufacturers. Search results may be sorted, filtered, and exported to Excel. Select Covered Entities to search to find a HRSA 340B ID.

  2. The Search button in the middle of the page allows searches for active covered entities, contract pharmacies, and manufacturers. Search results may be sorted, filtered, and exported to Excel. Select Covered Entities to search to find a HRSA 340B ID.

3. From Covered Entities Search page-- type in a key word, or the text for which you want to search (e.g., Entity Name, City, State or Zip).

4. This will generate a list of all covered entities that match your search criteria. The 340B ID is listed in the first column.

5. When you click on an entity’s 340B ID in the search results table, the entity’s details are displayed. The entity’s status is reflected by the color of the page header:

  • Active (green)

  • To be terminated (yellow)

  • Terminated (red)

6. You can export a selected 340B ID or all 340B IDs displaying in the search by clicking Export Selected or Export All.
