This article provides details on how to find a HRSA (Health Resources and Services Administration) Contract Pharmacy ID.
Covered entities that elect to utilize contract pharmacy arrangements are required to register each contract pharmacy. Covered entities must register contract pharmacy arrangements online during an open registration period after a written contract is in place.
To find a Contract Pharmacy ID:
Navigate to https://340bopais.hrsa.gov/
The Search dropdown from the top panel of the page allows searches for active covered entities, contract pharmacies, and manufacturers. Search results may be sorted, filtered, and exported to Excel. Select Contract Pharmacies to search to find a HRSA Contract Pharmacy ID.
The Search button in the middle of the page allows searches for active covered entities, contract pharmacies, and manufacturers. Search results may be sorted, filtered, and exported to Excel. Select Contract Pharmacies to search to find a HRSA Contract Pharmacy ID.
From Contract Pharmacies search page-- type in a key word, or the text for which you want to search (e.g., Entity Name, City, State or Zip).
From here, there are two ways to locate the HRSA Pharmacy ID:
Option 1:
Click the checkbox next to the blue hyperlink 340B ID, and then click Export Selected or Export All. Selecting the box in the top header will select all of the check boxes shown within the search results.

Select all or select the contract pharmacy details. Check the boxes next to 340B ID, Entity Name, Pharmacy Details and Pharmacy Address. Then, click Export to generate an Excel list of all contract pharmacies for that entity.
Exporting the report will produce an excel file. Within the excel file, you can locate the pharmacy ID in the Pharmacy Details section. Use the CTRL+F function to search the address of your desired pharmacy to locate its corresponding pharmacy ID.
Option 2:
The second option to find the pharmacy ID is in the Contract Detail section. Search by the Pharmacy name at the top of the search results. Then click on the blue hyperlink pharmacy name.
The url at the top of the browser will include the pharmacy ID. Within the below example, 681 represents the pharmacy ID.
